To use the graphic, click on the left-hand menu to see a list of counties. Then choose your county. You can visually see how the facilities in each county measure up or choose facilities to compare.

The spreadsheet belows shows what facilities had nursing home violations. Use it to search for specific facilities or to review violation reports and fines.
